I, the undersigned (the “Participant”) understand and acknowledge that in participating in dance, exercise and other activities hosted by Los Gatos Ballet or BIM Inc. I may be exposed to a variety of hazards and risks, foreseen or unforeseen. These inherent risks include, but are not limited to, personal injury or illness, property damage and other hazards of physical activity and any damages resulting there from (collectively, the “Injuries and Damages”). I accept and voluntarily incur all risks of any Injuries or Damages which may arise during or as a result of my participation in online classes. I understand these risks are involved in participation and acknowledge that I may have to exercise extra care for my myself and for others around me. I agree to waive, discharge and release Los Gatos Ballet, BIM Inc., its owners, officers, directors, employees, agents, leaders and contractors from any and all liability to the student caused by or resulting from the student’s participation in any class or program. This agreement is freely and voluntarily executed.

I do hereby consent to and authorize the use and reproduction by Los Gatos Ballet of any photographs or recordings of me or my child, for any advertising or promotional purpose. Such use includes the display or distribution of photographs, images, and/or video that include, but may not be limited to, printed materials (brochures, posters or other print advertising), digital materials (website, e-newsletters, videos or blogs), and social media. I furthermore understand that recording of online classes is strictly prohibited and agree not to share, reproduce or distribute any material from class recordings.
